Care Coordination Department opens at CMMC

Central Montana Medical Center is pleased to announce the opening of the new Care Coordination Department.

The goal of the Care Coordination Department is to achieve better health and better healthcare. The Care Coordination Department is staffed by two Care Coordinators, Dana Smart, RN, BSN and Pattie Carr, MA, RDN, CSSD.

Dana is a Registered Nurse with 17 years of clinical experience in cardiac care and trauma. Dana is a Lewistown native. Pattie is a Registered Dietitian and Exercise Physiologist who taught in higher education for the past 26 years. Pattie is originally from Kalispell and thrilled to be back home in Montana.

The Care Coordinators have many years of healthcare experience and are excited about using that experience to serve the community of Central Montana.

The purpose of the Care Coordination Department is to help people improve and/or maintain their health. Care Coordination is centered on the individual and the family. The objective of Care Coordination is to unite the various members of the healthcare team for the purpose of meeting the healthcare goals and needs of the person.

The Care Coordinators work in collaboration and continuous partnership with individuals and their families/caregivers, clinic/hospital/specialty providers and community resources.

The Care Coordinators strive to help people live healthier lives in many ways. We help people fill out health related paperwork. We help people find professionals to provide education about chronic diseases such as diabetes, cardiovascular disease and obesity. We help people get socially connected through clubs and organizations. We provide a wide range of health related resources.

The Care Coordinators help people navigate the healthcare system. We meet with people to answer health related questions and to facilitate a plan that encourages improved health.

This new department is being funded, in part, by a grant from Health Resources and Services Administration. The grant was written by Doris Barta, MHA, project director.

The Care Coordination Department is located in the Central Montana Medical Center in what was once the Emergency Room. The newly remodeled offices are accessible.
